Position Overview

We are seeking a passionate and dedicated Indigenous Case Manager to join our team at Inn from the Cold. The Indigenous Case Manager for the Moccasin Pathways program supports Indigenous families transitioning from homelessness to housing and independence. They provide intensive case management, advocacy, and resource navigation, working from an Indigenous worldview and Housing First approach.

 

What You’ll Do

  • Establish initial contact and conduct intake assessments with Indigenous families referred to the program, ensuring a thorough understanding of their needs and circumstances.
  • Guide and support families in securing suitable housing, including assisting with online searches, communicating with landlords, and attending housing viewings to ensure compatibility.
  • Foster ongoing communication and collaboration with landlords and community housing providers to address tenancy issues, facilitate rent payments, and explore subsidy opportunities for families.
  • Collaborate closely with families to develop personalized case plans, identifying achievable goals and strategies to overcome barriers, while embracing a Housing First and Indigenous Family Harm Reduction Model.
  • Provide consistent and compassionate one-on-one support to families, offering referrals, advocacy, and practical assistance to address various challenges related to housing stability and meeting basic needs.
  • Cultivate a culturally responsive approach by facilitating connections to Indigenous worldview-based activities and programs that promote family well-being and resilience.
  • Maintain client records and documentation, ensuring that case notes are accurately entered into relevant systems in a timely manner to track progress and facilitate communication within the organization.
  • Proactively research and share information about community resources, programs, and opportunities that may benefit client families, contributing to the agency's Community Resource Dashboard.
  • Participate in various meetings, trainings, and collaborative activities, including regular case consultations, team meetings, and organizational events, to enhance professional development and promote effective teamwork.

 

This position description reflects the general duties of the role, but is not intended to include all duties that may be inherent in the position. The Indigenous Case Manager may be asked to perform other duties as required or assigned.

 

 What You Bring to Our Mission

  • Completion of a diploma or degree program related to social work or human services, or an equivalent of experience in the human service sector.
  • Knowledge and understanding of Indigenous cultures and worldviews, with preference given to lived Indigenous experience.
  • 2 years’ experience as a case manager and familiarity with family-specific challenges, barriers, and resources.
  • Direct experience working with Treaty 7 Nations and Indigenous communities in Calgary is an asset.
  • Strong computer skills and knowledge of Microsoft Office Suite and experience using database systems for client documentation and record keeping.
  • Familiarity with Housing First, Harm Reduction, Trauma-Informed Care, and adherence to organizational policies and codes of conduct.
  • Valid class 5 driver’s license and access to a reliable vehicle, with a minimum $2,000,000 third-party liability insurance for a personal vehicle.
  • Capability to work with individuals experiencing behavioural and emotional challenges, employing conflict resolution skills.
  • Effective written and verbal communication, with the ability to work independently and in teams.
  • Proficient time management and organizational skills, adaptable to changing environments.
  • Creative problem-solving skills and commitment to fostering a positive workplace culture.
